California Licensed Care Management Nurse

Job Summary

A company is looking for a part-time Remote Chronic Care Management/Remote Therapeutic Monitoring Care Management Nurse.

Key Responsibilities

• Conduct telephonic encounters with patients and develop detailed care plans
• Manage a patient panel of a minimum of 100 patients per month, completing encounters on 90% of assigned patients
• Participate in mandatory meetings to define the current scope of work

Required Qualifications

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
• Current license to practice as an RN/LVN/LPN with no disciplinary actions
• Minimum of two years of clinical experience in Med/Surg, Case Management, or home health care
• Hands-on experience with Electronic Medical Records and Microsoft Office applications
• High-speed internet and a desktop or laptop computer with Windows 10 or higher or Mac
