California Licensed Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend plans for optimal care outcomes
- Develop and collaborate on ongoing care plans with providers and community resources
- Monitor member progress and update care plans as necessary to meet goals
Required Qualifications
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2 - 4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Must reside in California for remote position
