California Licensed Care Manager

Job Summary

A company is looking for a Care Manager responsible for managing at-risk patient populations and coordinating complex case management.

Key Responsibilities

• Conduct health risk assessments and develop individualized care plans for complex case management patients
• Coordinate care transitions and collaborate with healthcare providers, patients, and caregivers to facilitate effective discharge planning
• Manage a caseload of complex and rising risk patients, ensuring adherence to clinical criteria and documentation standards

Required Qualifications, Training, and Education

• High School Diploma or GED equivalent
• BSN degree from an accredited university
• Minimum of 2 years of case management experience in a managed care environment
• Minimum of 1 year of complex case management experience
• Registered Nurse state licensure and/or compact state licensure required; CCM certification preferred
