Job Summary

A company is looking for a Care Manager Transition of Care (State).

Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation
Develop care/service plans and collaborate with healthcare teams to support member transitions
Document care management activities to ensure compliance with regulations and provide feedback for quality improvement

Required Qualifications

Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of Nursing
2-4 years of related experience
Required licenses include LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
