California Licensed Care Manager

Job Summary

A company is looking for a Remote Care Manager.

Key Responsibilities

• Monitor inpatient and outpatient care services related to mental health and substance abuse treatment
• Provide telephone triage, crisis intervention, and perform concurrent reviews for care levels
• Advocate for patients and develop discharge plans in collaboration with providers and facilities

Required Qualifications, Training, and Education

• Current licensure required: CEAP, LMSW, LCSW, LPC, or RN
• Minimum 2 years of experience in healthcare, behavioral health, or substance abuse settings
• Knowledge of utilization management procedures and mental health resources
• Experience in both inpatient and outpatient settings
• Education level dependent on certification, ranging from Associate's to Master's in relevant fields
