Job Summary

A company is looking for a Lead Care Manager.

Key Responsibilities

• Assess member needs across various health and social domains and develop care plans
• Coordinate health care benefits and facilitate access to services for clients
• Advocate for clients with health care professionals and maintain accurate health records

Required Qualifications

• 2+ years of experience as a care manager, care navigator, or community health worker
• Experience working in a remote setting
• Knowledge of government and community resources related to social determinants of health
• Ability to work Monday-Friday, with potential evenings and weekends
• Bilingual (English/Spanish) preferred
