California Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluates member needs through post-discharge assessments and medication reconciliation
• Develops care/service plans in collaboration with healthcare teams to support member transitions
• Facilitates communication and services between members, providers, and resources to meet medical and socio-economic needs

Required Qualifications, Training, and Education

• Must reside in California
• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2 - 4 years of related experience in care management
• Licensure such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Experience with Medi-Cal and post-discharge care coordination is preferred
