California Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care (RN).

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate communication and services between members, providers, and resources to meet medical and socio-economic needs

Required Qualifications, Training, and Education

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2 - 4 years of related experience in care management
• Valid RN license or relevant behavioral health certification (LISW, LCSW, LMSW, LMFT, LMHC, LPC)
• Candidate must reside in California
