California Licensed Care Manager

Job Summary

Coordinating care for members, the full-time California Licensed Care Manager will work remotely to ensure high-quality, timely, and coordinated services while authorizing necessary care and collaborating with the Integrated Care Team.

Key responsibilities

• Serve as the primary contact for patients, families, and providers regarding care coordination and authorizations
• Ensure timely delivery of inpatient and outpatient services, adhering to policies and procedures
• Coordinate discharge needs and develop treatment plans based on evidence-based criteria and clinical judgment

Required qualifications

• Associate's Degree or equivalent experience
• 3-5 years of related experience
• California LVN license
• Ability to perform work under direct supervision
• Experience in providing leadership and guidance to care coordinators
