California Licensed Care Manager

Job Summary

Providing care coordination for members, the full-time California Licensed Care Manager will ensure high-quality, timely, and coordinated services while working remotely and covering non-business hours.

Key responsibilities

• Serve as the primary contact for patients, families, and providers regarding care concerns, transfers, and authorizations
• Coordinate necessary inpatient and outpatient services to ensure members receive appropriate care at the right time and location
• Review and manage care plans, discharge needs, and appeals while collaborating with the Integrated Care Team

Required qualifications

• Associate's Degree or equivalent experience
• 3-5 years of related experience
• Current California LVN license
• Ability to work under direct supervision and handle basic issues independently
• Experience in providing leadership and clinical guidance to care coordinators
