California Licensed Care Manager

Job Summary

Providing after-hours care coordination, the full-time California Licensed Care Manager will ensure high-quality and timely services for members by collaborating with the Integrated Care Team, authorizing necessary services, and addressing patient needs remotely.

Key responsibilities

• Serve as the primary contact for patients, families, and providers regarding care concerns, transfers, and authorizations
• Coordinate discharge needs from inpatient settings and ensure appropriate services are delivered to members
• Review and manage care tasks assigned by Integrated Care Nurse Practitioners, including home health and outpatient services

Required qualifications

• Associate's Degree or equivalent experience
• 3-5 years of related experience
• California LVN license
• Ability to work under direct supervision and handle basic issues
• Experience providing leadership and mentoring to care coordinators
