California Licensed Care Manager
Job Summary
To enhance the ability of frail seniors to age in place, the full-time California Licensed Care Manager will provide telephonic person-centered care, conduct assessments, and develop care plans while working remotely during Pacific Time business hours.
Key responsibilities
- Perform initial and annual telephonic assessments for dually eligible members to determine care needs
- Develop and implement member-centered care plans based on clinical guidelines and community standards
- Provide education and coaching for chronic condition management to promote member engagement and self-management
Required qualifications
- Associate's degree in nursing (ASN) required; Bachelor's degree in nursing (BSN) strongly preferred
- CA Registered Nurse (RN) license required
- 3+ years of experience in managed care, healthcare, or case management
- Bilingual in English and Spanish preferred
- Strong understanding of local community resources for seniors
