Job Summary
To support post-discharge members, the full-time California Licensed Care Manager will perform care management duties, including assessing, planning, and coordinating medical services remotely.
Key responsibilities
- Evaluates member needs through post-discharge assessments and medication reconciliation
- Develops care/service plans in collaboration with discharge planners and interdisciplinary teams
- Facilitates transitions into active care management and provides education to members and caregivers
Required qualifications
- Active RN license in the state of California
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
- 2 - 4 years of related experience in case management or transitional care
- Preference for candidates residing in California and able to work Pacific Time (PST) hours
- Relevant certifications such as LISW, LCSW, LMSW, LMFT, LMHC, or LPC are preferred
