California Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) (State).

Key Responsibilities

• Develops and facilitates personalized care plans for members with complex physical needs
• Coordinates care between members, families, and providers to ensure effective service delivery
• Monitors member progress and updates care plans as necessary to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
