California Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) to manage complex patient care and coordination remotely.

Key Responsibilities

• Conduct risk stratification and develop individualized care plans for at-risk patient populations
• Coordinate care transitions and collaborate with healthcare teams to ensure effective discharge planning
• Manage a caseload of complex and/or rising risk patients, providing telephonic and onsite case management as needed

Required Qualifications, Training, and Education

• High School Diploma or GED equivalent
• BSN degree from an accredited university
• Minimum of 2 years of case management experience in a managed care environment
• Minimum of 1 year of complex case management experience
• Licensure as a Registered Nurse (RN) in the applicable state
