California Licensed Care Manager (RN)
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
- Develop ongoing care plans and collaborate with providers to meet members' unmet needs
- Provide resource support and education to members and their families regarding healthcare options and benefits
Required Qualifications
- Must reside in California
- Current California RN - Registered Nurse - State Licensure
- Experience with the Medi-Cal population
- Experience in telephonic assessment
- Degree from an accredited school of nursing or a Bachelor's degree in Nursing with 2-4 years of related experience
