Job Summary

A company is looking for a Care Manager RN - Part-time - REMOTE.

Key Responsibilities

• Carry out effective case management functions for complex patients and their families
• Communicate daily with medical staff and outside resources to ensure appropriate resource utilization
• Monitor medical records for variances and intervene to achieve optimal patient outcomes

Required Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire
• California Registered Nurse License upon hire
• 2 years of acute care experience in a Medical Surgical or Inpatient setting
• 1 year of experience in care management or utilization review, or completion of TIPS or Case Management Orientation Program upon hire
