California Licensed Care Manager RN

Job Summary

To enhance frail seniors' ability to age in place, the full-time temporary California Licensed Care Manager RN will provide telephonic person-centered care, conduct assessments, and develop care plans while working remotely during Pacific Time business hours.

Key responsibilities

• Perform initial and annual telephonic assessments for dually eligible members and determine Nursing Facility Level of Care
• Conduct Enhanced Care Management activities for specific populations at risk of hospitalization or institutionalization
• Participate in interdisciplinary planning and case conferences to ensure comprehensive and effective member-centered care

Required qualifications

• Associate's degree in nursing (ASN) required; Bachelor's degree in nursing (BSN) strongly preferred
• CA Registered Nurse (RN) license required
• 3+ years of experience in managed care, healthcare environment, or case management
• Bilingual in English and Spanish preferred
• 2+ years of experience working with seniors and remotely strongly preferred
