California Licensed Case Manager RN

Job Summary

A company is looking for a Case Manager Nurse RN.

Key Responsibilities

• Conduct thorough assessments of patients' needs and develop treatment plans
• Monitor interventions, evaluate treatment effectiveness, and maintain communication with patients and healthcare providers
• Document case management activities and perform Utilization Review for assigned members

Required Qualifications

• Graduation from an accredited Registered Nursing (RN) program
• Current California RN license; multi-state license preferred
• Minimum of five years of medical/surgical or acute care experience, including two years in case management
• Prior experience in emergency room, critical care, or relevant clinical care
• Knowledge of medical claims and coding (ICD-10, CPT, HCPCS)
