Job Summary

A company is looking for a Case Manager Nurse-RN.

Key Responsibilities

• Conduct thorough assessments of patients' needs and develop treatment plans in collaboration with healthcare providers
• Monitor treatment effectiveness and maintain communication with patients, families, and multidisciplinary teams throughout care
• Document case management activities and perform utilization reviews while ensuring confidentiality and compliance with policies

Required Qualifications

• Graduation from an accredited Registered Nursing (RN) program
• Current California RN license; multi-state license preferred
• Minimum of five years of medical/surgical or acute care experience, including two years in case management
• Prior experience in emergency room, critical care, or relevant clinical care
• Knowledge of medical claims, coding, and community resources
