Job Summary

A company is looking for a Registered Nurse, Case Manager - Remote.

Key Responsibilities

• Conducts telephonic comprehensive nursing assessments and creates care plans
• Coordinates care with patients, families, and healthcare teams to ensure effective treatment plans
• Maintains documentation and adheres to departmental policies and HIPAA standards

Required Qualifications

• Graduation from an accredited school of nursing
• Active, unrestricted Registered Nurse license in California
• 1+ years of experience in a clinical setting or care management
• Intermediate level experience in Microsoft Office products
• Ability to work PST business hours
