Job Summary

Seeking a motivated California Licensed Chronic Care LVN, the full-time remote position will manage high-volume outbound patient calls, conduct comprehensive assessments, and educate patients on care plans while ensuring effective communication and documentation in a clinical call center environment.

Key Responsibilities
  • Conduct high-volume outbound calls to check in on patients, addressing health concerns and maintaining a call volume of 70 to 90 calls per day
  • Document all patient interactions in real-time and collaborate with providers to ensure timely and effective patient care
  • Adapt to fluctuating call volumes and shifting priorities while maintaining high standards of patient care and compliance with company policies
Required Qualifications
  • Active LPN/LVN Compact License required
  • Active, unrestricted LVN license issued by the California BVNPT
  • Strong passion for patient care and teaching
  • Proficiency with computers, EMRs, and telehealth tools
  • Previous experience in a call center or high-volume phone-based work environment preferred
