California Licensed Chronic Care LVN

Job Summary

To support a growing Chronic Care Management team, the full-time California Licensed Chronic Care LVN will conduct high-volume outbound patient calls, perform assessments, and assist with care plans in a remote clinical environment.

Key Responsibilities

• Manage inbound and outbound telephonic patient interactions, maintaining a call volume of 70 to 90 calls per day
• Conduct and document monthly Chronic Care Management care plan calls, ensuring timely outreach and comprehensive care coordination
• Collaborate with providers to coordinate effective patient care through telephonic communication and accurate real-time documentation

Required Qualifications

• Active LPN/LVN Compact License required
• Active, unrestricted LVN license issued by the California BVNPT
• Passionate about patient care with the ability to relate with empathy and compassion
• Proficient with computers, EMRs, and telehealth tools
• Strong communication and organizational skills
