Job Summary

A company is looking for a Community Health Worker (CHW) Coordinator.

Key Responsibilities

• Submit, track, and maintain health plan authorizations while ensuring timely updates
• Conduct outreach and welcome calls to newly enrolled members and coordinate with case-carrying CHWs
• Ensure accurate documentation and compliance in care management systems and identify training needs

Required Qualifications

• CHW certification or equivalent lived/field experience preferred
• 2-4+ years of Community Health Worker or ECM-related experience
• Strong knowledge of ECM models and care coordination workflows
• Experience with Care Director, Pathways, or similar care management systems preferred
• Ability to work independently and manage competing priorities
