Job Summary

A company is looking for a California Licensed Clinical Assessment Manager.

Key Responsibilities

Coordinate with the clinical team for OASIS review and plan of care development
Educate staff on OASIS documentation and conduct case conferences to discuss care plans
Review patient cases and assist clinicians in establishing therapeutic goals and care plans

Required Qualifications

Registered nurse with current licensure to practice in California
OASIS certification required prior to starting the position
Minimum two years of experience conducting quality OASIS reviews in a home health setting
Graduate of a National League for Nursing accredited school; BSN preferred
Experience in star measures and value-based purchasing
