Job Summary

Key Responsibilities

Assess member needs across various health and social support areas
Oversee the development of client care plans and facilitate access to care
Advocate for clients with healthcare professionals and maintain documentation

Required Qualifications

Active LCSW, LMFT, or LPCC license in California
1+ year of experience in care management or related fields
Knowledge of government and community resources for social determinants of health
Availability to work Monday-Friday, with potential for evenings and weekends
Bilingual (English/Spanish) preferred
