California Licensed Clinical Case Manager

Job Summary

A company is looking for a Clinical Case Manager.

Key Responsibilities

• Assess member needs and oversee the development of client care plans
• Advocate for clients with healthcare professionals and connect them to necessary social services
• Maintain patient health records and complete documentation within established timeframes

Required Qualifications

• Active, unrestricted LCSW, LMFT, or LPCC license in California required
• 1+ year of experience as a care manager, care navigator, or community health worker
• Working knowledge of government and community resources related to social determinants of health
• General computer skills and familiarity with Google Workspace and MS Office
• Bilingual (English/Spanish) highly preferred
