California Licensed Clinical Case Manager

Job Summary

Seeking a compassionate California Licensed Clinical Case Manager, the full-time position will coordinate care, advocate for patients, and assist with outpatient discharge planning in an outpatient ambulatory clinic setting.

Key responsibilities:

• Collaborate with healthcare providers, payers, and community resources to ensure continuity of care
• Serve as a patient advocate, facilitating access to necessary services and resources
• Maintain accurate documentation in the EPIC electronic medical record system

Required qualifications:

• Bachelor's Degree in Nursing (BSN)
• Current California Registered Nurse License
• Minimum of three (3) years of clinical experience in Case Management, preferably in an outpatient setting
• BLS Certification through the American Heart Association
• Experience with cancer treatment protocols is preferred
