California Licensed Clinical Case Manager

Job Summary

To ensure seamless, person-centered care for participants in Intensive Outpatient Programs, the part-time California Licensed Clinical Case Manager will manage patient engagement, care coordination, and compliance documentation while collaborating with interdisciplinary teams in a remote setting.

Key Responsibilities

• Conduct IOP orientations and use motivational interviewing to enhance patient retention and engagement
• Lead discharge planning and ensure linkage to outpatient providers and community resources
• Document case management activities in compliance with CMS and commercial payor standards

Required Qualifications

• 3-5 years of experience as a Clinical Case Manager or licensed practitioner
• Current clinical licensure in California
• Strong knowledge of evidence-based treatments for mood, anxiety, and trauma
• Proficiency with telehealth platforms and remote clinical work
• Licensed Marriage and Family Therapist (MFT) or Licensed Clinical Social Worker (LCSW) in California
