California Licensed Clinical Documentation Specialist

Job Summary

Conducting concurrent and retrospective reviews of patient medical records, the full-time California Licensed Clinical Documentation Specialist will evaluate the accuracy and completeness of clinical documentation while collaborating with healthcare teams to optimize reimbursement and enhance outcome reporting in a remote setting.

Key responsibilities

• Review patient medical records to ensure documentation accurately reflects the severity of illness and quality of care
• Interact with physicians and care teams to gather and clarify medical record documentation
• Identify opportunities for improvement in documentation practices and reimbursement processes

Required qualifications

• Graduate of an accredited school of nursing or foreign-trained physician
• Current RN license or foreign-trained physician status
• CDIP or CCDS certification
• Minimum of 5 years of recent relevant experience
• Strong knowledge of coding classifications and reimbursement guidelines
