Job Summary
Conducting concurrent and retrospective reviews of patient medical records, the full-time California Licensed Clinical Documentation Specialist will evaluate the accuracy and completeness of clinical documentation while collaborating with healthcare teams to optimize reimbursement and enhance outcome reporting in a remote setting.
Key responsibilities
- Review patient medical records to ensure documentation accurately reflects the severity of illness and quality of care
- Interact with physicians and care teams to gather and clarify medical record documentation
- Identify opportunities for improvement in documentation practices and reimbursement processes
Required qualifications
- Graduate of an accredited school of nursing or foreign-trained physician
- Current RN license or foreign-trained physician status
- CDIP or CCDS certification
- Minimum of 5 years of recent relevant experience
- Strong knowledge of coding classifications and reimbursement guidelines
