California Licensed Clinical Intake Specialist

Job Summary

A company is looking for a Clinical Intake Specialist to join their Enhanced Care Management team.

Key Responsibilities

• Conduct outreach to newly referred clients and log activity in Salesforce
• Perform 8-12 intake visits daily, ensuring efficient assessments and care planning
• Review medical charts and communicate critical information to the care team

Required Qualifications

• Active, unrestricted California LVN license, Bachelor's degree in a related field, or Medical Assistant certificate
• Minimum of 2 years of intake or admissions experience in a high call volume environment
• Experience working with complex care clients and ECM populations
• Proficiency in Health Gorilla or health record systems
• Bilingual (English/Spanish) is highly preferred
