California Licensed Clinical Intake Specialist
Job Summary
To support a growing Enhanced Care Management team, the full-time California Licensed Clinical Intake Specialist will conduct client outreach, perform intake assessments, and connect clients with appropriate resources while working remotely.
Key responsibilities
- Conduct outreach to newly referred clients and log activity in Salesforce
- Perform 8-12 intake visits daily, ensuring efficient assessments and care plan development
- Communicate critical client information to the care team for personalized care planning
Required qualifications
- Active, unrestricted California Licensed Vocational Nurse (LVN) license, Bachelor's degree in a related field, or Medical Assistant certificate
- Minimum of 2 years of intake or admissions experience in a high call volume environment
- Experience working with complex care clients and ECM Populations of Focus
- Proficiency in Health Gorilla or Health Record Systems
- Bilingual (English/Spanish) highly preferred
