California Licensed Clinical Outreach Specialist

Job Summary

A company is looking for a Clinical Outreach Specialist to connect with prospective clients and guide them through Enhanced Care Management services.

Key Responsibilities

• Engage with prospective clients and record interactions in the CRM system, making 60-100 outbound calls daily
• Convey essential information and client preferences to the care team for personalized care planning
• Assist clients in completing psychosocial assessments and developing actionable care plans

Required Qualifications

• Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate, or Bachelor's degree in a related field with 3+ years of direct healthcare experience
• Ability to work Monday through Friday, 8:30am-5:00pm Pacific Time, with flexibility for evenings and weekends
• Comfortable with technology, including video conferencing and electronic health records
• Prior experience in a remote role with a secure workspace
• Bilingual (English/Spanish) preferred
