California Licensed Clinical Pharmacist

Job Summary

A company is looking for a Clinical Pharmacist for the West Coast.

Key Responsibilities

• Execute pharmacist-led disease state management through collaborative practice agreements
• Maintain a patient panel for chronic condition disease management, including diabetes, HTN, CHF, and COPD
• Collaborate with providers and team members to ensure exceptional patient care

Required Qualifications, Training, and Education

• Active, unrestricted pharmacist license in the state of practice
• At least 3 years of pharmacy experience or ASHP-accredited PGY1 Residency
• Experience in ambulatory care, managed care, or other clinical settings
• Bachelor's degree or PharmD
• Willingness to obtain licenses in additional states as required
