Job Summary

A company is looking for a Clinical Pharmacist for remote work in California.

Key Responsibilities:

• Conduct pharmacist-led outreach to members, providers, and pharmacies to resolve medication-related barriers
• Complete medication therapy reviews and issue clinical recommendations for therapy optimization
• Document all outreach, interventions, and outcomes in specified platforms and track progress toward performance goals

Required Qualifications:

• Doctor of Pharmacy (PharmD) degree
• Active, unrestricted California pharmacist license
• Strong clinical knowledge in chronic conditions and evidence-based therapeutic management
• Proficiency in medication therapy management and adherence strategies
• Ability to work with population health platforms
