Job Summary

A company is looking for a Clinical Pharmacist for the West Region.

Key Responsibilities

• Conduct provider consultations to support medication-related changes and provide evidence-based recommendations
• Track drug usage and execute value-based care initiatives while coordinating patient drug therapy
• Collaborate with cross-functional teams to design and implement strategic initiatives that enhance care delivery

Required Qualifications

• Graduate of an ACPE-accredited PharmD program
• Current, clear, and unrestricted pharmacist licensure in relevant states
• Successful completion of a PGY1 pharmacy residency program or three years of pharmacy experience
• 3+ years of experience communicating medication recommendations in a telehealth setting
• Intermediate proficiency in MS suite and Epic EHR
