Job Summary

A company is looking for a Clinical Pharmacist, Multidisciplinary Inbasket Support Team (MIST) - Work From Home.

Key Responsibilities

• Collaborate with healthcare providers to ensure appropriate medication therapy for patients
• Perform refill assessments and prescribe medications under a collaborative practice agreement
• Provide clinical assessment regarding patient home readings and support medication-related inquiries

Required Qualifications, Training, and Education

• PharmD from an accredited pharmacy school
• Current registration as a pharmacist within 120 days
• Completion of ASHP Post Graduate Year 1 General Pharmacy Residency
• Preferred board certification and advanced practice pharmacist licensure
• At least 3 years of recent relevant experience
