California Licensed Clinical Pharmacist

Job Summary

To optimize drug therapy and outcomes in the Anticoagulation clinic, the part-time California Licensed Clinical Pharmacist will collaborate with healthcare providers and patients, providing effective ambulatory pharmaceutical care while working remotely within California.

Key responsibilities

• Participate in the evaluation and management of diseases in collaboration with other healthcare providers
• Prescribe and manage pharmaceutical agents according to the collaborative drug therapy management agreement
• Address medication-related issues to support care coordination and ensure patient access to safe and effective medication therapy

Required qualifications

• Graduation from an accredited school of pharmacy
• Active CA Pharmacist License in good standing
• Current knowledge of evidence-based clinical pharmacy practice and pharmacotherapies
• Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
• Demonstrates skills necessary to provide care appropriate to all age groups in an ambulatory pharmacy setting
