Job Summary
Delivering pharmaceutical care services at the patient and population level, the full-time California Licensed Clinical Pharmacist will analyze data to improve medication use, provide clinical services, and educate patients and providers while working remotely.
Key responsibilities
- Provide clinical services, medication literature evaluation, and drug information services in accordance with regulations
- Analyze large data sets to identify strategies that enhance medication use across the care continuum
- Implement patient care plans based on health status and risk factors, collaborating with healthcare teams
Required qualifications
- PharmD from an accredited pharmacy school
- Registered Pharmacist (RPH) license
- Minimum of 5 years of recent relevant experience
- Completion of a post-graduate year one (PGY1) residency or 3 years of experience in a managed care or health-system environment
- Knowledge of legal requirements and accreditation standards related to pharmacy practice
