Job Summary
As a key member of the multidisciplinary care team, the full-time California Licensed Clinical Pharmacist will provide evidence-based drug therapy recommendations, conduct comprehensive medication reviews, and collaborate with healthcare providers to improve health outcomes for high-risk seniors, working remotely and in the field about 25% of the time in Riverside, CA and surrounding areas.
Key responsibilities
- Collaborate with a multi-disciplinary team to manage chronic diseases through direct patient care and consultations
- Conduct comprehensive medication reviews for high-risk patients, recommending adjustments and optimizing treatment outcomes
- Identify barriers to medication access and collaborate with various stakeholders to ensure cost-effective therapy options
Required qualifications
- Graduate or Advanced Degree, Pharm D required
- Active California Pharmacist License required or willingness to obtain
- Minimum of two years of strong clinical pharmacy experience
- Strong clinical knowledge of chronic disease states such as diabetes, heart failure, and hypertension
- Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience preferred
