Job Summary

A company is looking for a Clinical Pharmacy Specialist who will enhance patient care through effective medication management in a remote setting.

Key Responsibilities
  • Provide comprehensive medication management, including reconciliation and dose optimization
  • Collaborate with healthcare teams to create individualized care plans and ensure medication coordination
  • Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications, Training, and Education
  • Doctor of Pharmacy (PharmD) degree
  • Licensed pharmacist in California
  • Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency with 1 year of ambulatory practice under a CPA
  • At least 3 years of ambulatory practice with disease state management
  • Strong organizational and technical skills
