Job Summary
A company is looking for a Clinical Pharmacy Specialist to enhance patient care through integrated specialty pharmacy services.
Key Responsibilities
- Provide comprehensive medication management, including reconciliation and dose optimization
- Collaborate with healthcare teams to create individualized care plans under a Collaborative Practice Agreement
- Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in California
- Completion of a PGY2 Ambulatory Care Residency, or PGY1 Residency with 1 year of ambulatory practice, or 3 years of ambulatory practice with disease state management
- Strong organizational and technical skills
- Empathy and a team-first mindset
