California Licensed Clinical Program Manager
Job Summary
A company is looking for a Clinical Program Manager RN - REMOTE - Days.
Key Responsibilities
- Lead the alignment and improvement of patient length of stay for designated populations
- Serve as a liaison with providers and Care Coordination teams to facilitate safe discharge plans
- Plan and develop reporting tools for information sharing between division and ministry teams
Required Qualifications
- Associate's Degree in Nursing
- Bachelor's Degree in Nursing
- California Registered Nurse License upon hire
- 5 years of experience as a utilization/case manager in an acute care setting (preferred)
- Experience in a multi-hospital and/or integrated healthcare system (preferred)
