California Licensed Clinical Social Worker

Job Summary

Seeking a dedicated California Licensed Clinical Social Worker for a per diem role, the Pre-Enrollment LCSW will conduct comprehensive psychosocial assessments and eligibility evaluations for prospective PACE participants, collaborating with an interdisciplinary team to ensure safe transitions into community-based care in a remote capacity.

Key responsibilities

• Conduct comprehensive psychosocial assessments and evaluate cognitive, emotional, and social functioning of prospective PACE participants
• Collaborate with the enrollment team to develop preliminary care plans and identify immediate service needs prior to enrollment
• Document all assessments and eligibility determinations in accordance with PACE regulatory requirements and organizational policies

Required qualifications

• Current, unrestricted Licensed Clinical Social Worker (LCSW) license in California
• Master's degree in Social Work (MSW)
• Minimum of 1 year of post-licensure clinical experience, with at least 2 years working with older adults
• Strong knowledge of geriatric mental health and experience conducting comprehensive psychosocial assessments
• Experience with standardized geriatric assessment tools and interpreting results for eligibility determination
