Job Summary
To support patient transitions, the per diem California Licensed Clinical Social Worker will manage communication with patients, health plans, and medical groups while providing excellent customer service in a remote environment during day shifts.
Key responsibilities
- Communicate effectively with patients and healthcare partners to facilitate care transitions
- Provide resources and support throughout the Providence continuum of services
- Exercise initiative and judgment to ensure follow-up and follow-through on patient care
Required qualifications
- Master's Degree in Social Work
- California Clinical Social Worker License or California Marriage and Family Therapist License upon hire
- Five years of previous experience in a healthcare role
- Practical experience in customer service
