Job Summary

A company is looking for a Maternity Care Navigator / Community Doula - Bilingual.

Key Responsibilities:

• Conduct outreach, assessments, and assist patients with appointment setting and social services applications
• Coordinate care services and advocate for patient needs with health plans and multi-disciplinary teams
• Engage with patients using evidence-based practices to promote positive habit changes

Required Qualifications:

• 1-2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, or Social Worker
• Strong preference for candidates with lived experience as a mother or working with mothers
• Comfortable using technology such as Google Workspace, Notion, Slack, and Zoom
• Fluency in English and bilingual in Spanish, Vietnamese, Tagalog, Cantonese, Dari, or Haitian Creole
• Located in California
