California Licensed Community Health Worker

Job Summary

To support community health initiatives, the full-time California Licensed Community Health Worker Team Lead will supervise and manage a team of Promotores de Salud, ensure quality documentation, and coordinate program events while working remotely within Santa Clara, Santa Cruz, or San Francisco counties.

Key responsibilities

• Oversee the performance and development of the Promotora de Salud team through regular evaluations and training
• Ensure timely and accurate documentation of team activities and implement quality assurance measures
• Coordinate community events and support administrative activities related to program changes

Required qualifications

• Bachelor's Degree or equivalent experience in social services
• 3+ years of experience in a healthcare setting
• 2+ years of supervisory experience with frontline staff
• Experience working with Medicaid/Medicare and directly with patients
• Fluency in English and Spanish, both verbal and written
