Job Summary
To support community health initiatives, the remote California Licensed Community Health Worker will assist patients and families in navigating services, advocate for health needs, and provide direct services such as screenings during outreach activities.
Key responsibilities
- Assist patients with accessing community services and provide culturally appropriate health education
- Conduct follow-ups with patients, including home visits and assistance with applications for services
- Collaborate with medical providers to ensure comprehensive and coordinated patient care
Required qualifications
- High School Education/GED or equivalent; Associate's/Technical Degree preferred
- Successful completion of a Community Health Worker formal training program from an accredited institution
- Experience in providing health education and support to diverse populations
- Ability to establish trusting relationships with patients and their families
- Familiarity with community resources and services for health-related needs
