California Licensed Complex Care RN

Job Summary

To support high-acuity Medicare Advantage members, the full-time California Licensed Complex Care RN will coordinate care, manage transitions of care, and ensure timely clinical monitoring in a fully remote environment.

Key responsibilities:

• Own the member journey by maintaining engagement and coordinating care activities for a panel of medically complex members
• Manage transitions of care and hospital discharge coordination to reduce readmissions and ensure safe community reintegration
• Conduct medication reconciliations and monitor clinical status, escalating concerns as needed to support effective care delivery

Required qualifications:

• Minimum 3 years of clinical RN experience in complex care, care management, or related settings
• Active, unrestricted Registered Nurse (RN) license in applicable state(s)
• Experience in a Medicare Advantage or managed care environment with knowledge of HEDIS and care gap management
• Proficiency in telehealth delivery and virtual member engagement
• Associate Degree in Nursing (ADN) with a Bachelor of Science in Nursing (BSN) strongly preferred
