Key Responsibilities

Initiate and participate in multidisciplinary discharge rounds as needed

Identify and track members at high risk for hospital admission/readmission

Required Qualifications

Active RN License
5+ years' experience in healthcare/health insurance, with 2+ years in case management preferred
Capacity to support program development and change
Strong clinical documentation skills
